We are providers for the following vision insurance plans. If your insurance provider is not listed below, please contact us, as this is a general list of common providers. Please have your insurance information ready when scheduling an appointment by phone. We would like to remind you that vision insurance and medical insurance are two separate entities. If you have any questions, please contact your Human Resources department before providing us with your insurance information.
Health Maintenance Organization (HMO)
HMO (Health Maintenance Organization) A type of health insurance plan that usually limits coverage to care from doctors who work for or contract with the HMO. It generally won't cover out-of-network care except in an emergency.
An HMO may require you to live or work in its service area to be eligible for coverage. HMOs usually provide integrated care and focus on prevention and wellness.
